• If you requested Terms and Conditions: Our team is reviewing your request and will either approve it or reach out with next steps.
• If you requested to pay by credit card: Expect a call from our team shortly to confirm your payment details securely.

Where to Start on the Website

• Chain of Custody (CoC) Forms → Download and complete before sending in samples. You can find CoCs under Client Resources in the menu.
• Test Codes & Services → Review available analyses and confirm which codes match your testing needs.
• Portal Access → Be on the lookout for an invitation email from QBench to set up your secure client portal. This is where you’ll track projects, view results, and manage records.

• Supplies → Order your sampling supplies and shipping labels on our easy to use store. The store is password protected and once entering, you should not need to enter again. Please check your email for the password.
